The incident happened around 5:39 p.m. at the intersection of 2140 Road and 850 Road, approximately 10 miles north of Butler.

According to the OHP report, Curtis D. Walker had exited the tractor when he realized the vehicle was still in drive. Troopers said Walker attempted to jump back into the cab, but fell on the step of the tractor. The attached trailer then ran over him.
